What is Community Support
Team?
The service is delivered by a three
member team of qualified professionals and additional direct support
personnel.
The service may include, but is not limited to…
- Crisis Management
- Case Management
- Mental Health Awareness
- Skills Training
Service Delivery
The service is delivered by a three member team of qualified
professionals & additional direct support personnel. This particular
service is delivered to the adult in both their home & out in their
local community. They will ensure linkage to needed community
services & resources, provide self help and living skills training,
and provide training that helps the person build skills for coping
with their mental health disorder. The team will also monitor and
manage the presenting psychiatric and/or addiction symptoms and work
with caregivers in the implementation of home based behavioral
supports.
If needed, A Caring Alternative will arrange a psychological
assessment prior to being admitted into service. If the adult has a
mental health disorder but does NOT meet entrance criteria for
Community Support Team Services, A Caring Alternative can and will
provide other service(s), such as outpatient therapy, case
management, and/or a 1:1 worker as long as:
- the person does not have these
services already open to another mental health provider,
- the person meets the
state-defined entrance criteria for these services, and
- the person consents to and has
a desire to receive the service.
The CST provider is an advocate,
broker, coordinator, and monitor of the service delivery system on
behalf of the person. If the person makes enough progress to where
this level of service is no longer needed, the team will discharge
the person and refer them into a less intensive service, such as
outpatient therapy and/or standard community support services.
